Claude 16c61ed3ab perf: stream napplet files and build the shell in one pass
- The shell base64-encodes the prelude and the napplet straight into
one presized array, carrying 0-2 bytes between parts: a 20 MB napplet
now allocates only its 28 MB page instead of several copies of it.
- The prelude is built once per launch and prepended as bytes.
- Scripts, wasm and images stream from the cache file instead of being
read whole into memory; Android's WebView and the desktop's Chromium
both read the response as a stream, and a cancelled desktop request
  closes it.
- Concurrent misses on one blob share one download.
- A cache hit touches the file at most hourly, so trimming evicts the
least recently used blobs, not the oldest downloads.
- Downloads that already verified their hash are stored without
  hashing them again.
